Because the mouthpiece breaks twice, it puts pressure on the tongue and bars of the horse's mouth. 
The twist in the bit provides a little bit of a “bite” as compared to a smooth mouthpiece. The 3 piece twisted, medium shank with a locked mouthpiece is one of the top 2 bits sold over the decades. It is popular because the twisted mouthpiece provides a little bit more control and the medium shank provides more leverage and “whoa” as compared to the short shank and not quite as much compared to the long shank.
